To be a volunteer on a board, please complete the application and the other four documents and send all of the documents in to the CRB office. You have the option to do one of the following:

If you choose to mail or fax the information to us simply print the required documents and send them to our office. If you wish to send them via email, after you've completed the documents you can scan and send them to us.
